Group and Individual Consultation Signup Notes:

• You will be assigned to a Cohort (A, B, C, etc.). Be mindful of your Assigned Cohort and the Group(s) you sign up for, as Group Slots will be labeled "Group - COHORT A," "Group - COHORT B," etc. We will be keeping Cohorts in the same groups, but Individual consultation slots will be first come, first served.

• During this certification period, you will be completing Group consultation for 10 hours (in Five 2-hour groups), and the remaining 10 hours will be Individual. Plan for One 2-hour group consultation monthly. Pay attention to the Date/Time as you're signing up and add to your personal calendar for reminders

• Reminders may go out automatically to the email you use to sign up, but please also record the date/time for each consult in your own calendar as you are responsible for attending as scheduled.
• You may also sign up for text reminders.
• If you sign up for a 1:1 consultation and realize you cannot attend, please cancel your registration as soon as you're able so that someone else may take the slot, then register for another available day/time. (If you miss a consultation, please see the fees below for missing a group or individual consultation).

**All group and 1:1 Consultations must be completed before 11/30/2025 to allow time for any final Admin tasks to be completed and submitted to PITT.**

• Check back on my sign-up page weekly or email me if you don't see a slot available as I'll continue updating it with additional days/times for Groups and 1:1 for future months. Please make sure you only sign up for the required number of slots you need (Five 2-hour group slots, Ten 1:1 slots). It should take 10 months to complete this process.

FEES for Late Cancel / No-Show of Consultation

• A late cancellation is canceling less than 24 hours before your scheduled Consultation Slot. A No-show is not showing up for your consultation at all. 2 Missed Consultations without notice will result in certification slots being placed on hold and assignment to a later cohort.
• Late Cancel/Missed Consult Fee for Individual: $100 per consultation
• Late Cancel/Missed Consult Fee for Group: $50 per consultation
